Evidence

Before a3f65be, the section said what each entry holds: "The list
names each member by a lowercase hyphenated name, with its path from the
umbrella root and its role." That commit dropped the sentence, which
leaves the placeholder tokens as the only explanation of the form. Once
an umbrella fills the list, as the validator requires, the explanation
disappears.

Scenario

Take an entry such as - app-api: `AppAPI`, `contribute` . An agent
has to work out which field is the <name> for
.worktrees/<name>/<branch> (line 387) and which is the <path> for
git -C <path>. Where a name differs from its path only in case, the
agent can open .worktrees/AppAPI/<branch>, and later sessions then
find no worktree under the expected name.

Smallest fix

Restore a one-line legend before the list, for example: "Each entry
gives a member's lowercase hyphenated name, its path from the umbrella
root, and its role."
