Why

The upgrade guide omitted evals/ from its source-only paths, so agents
could copy maintainer tooling into adopted repositories or stop to ask.
The forge-template cleanup instructions had the same omission.

What changed

Upgrade instructions now keep root evals/ in the source, and new
repository instructions remove it after a forge template copy. Both
paths match the README's existing adoption scope.

How to verify

Read the source-only list in upgrade step 3 and the cleanup list under
New repository: both now include root evals/.

All Maintenance checks passed: 268 agent configuration tests, 46
work-folder tests, ShellCheck (53 files), Prettier, the static
configuration validator, and git diff --check main. Document review
passed. Paid behavioral evals were not run for this prose correction.
