Three-way merge of Agent Foundry d9a687b..e9006dd, which adds the
forge skill: sessions open pull requests and create issues through gh
or fj under their own forge identity, and fall back to text ready to
paste when neither CLI holds a login for the remote's host.

AGENTS.md merged cleanly. The end-of-work choice gains "push and open
a pull request", offered only when the forge check passes. Creating an
issue joins the actions taken only on request. Reviewers list defects
outside the change separately, and the content rule now covers
published text as well as commits. The forge skill is linked for
Claude Code under .claude/skills, and the foundry-upgrade Record points
at the new commit. No migrations applied.
